CARE PLAN GENERATORwrites the first draft

Care plans that reach the visit.

Keyed to the code you bill. Signed with a certificate. On the caregiver's phone on the right day.

Where the plan travels

The plan, the visit and the claim agree.

One service code carries from the authorization to the remittance. Nothing is re-typed on the way.

One code. Five artifacts.

The plan underneath

Built from the authorization, not from a blank page.

A task library keyed to your billing codes seeds the plan. You edit it; it never overwrites you.

One line, one plan

Each authorization line gets its own plan, grouped by service code.

Day-exact in the shift

The caregiver opens the shift and sees this client's tasks for this day.

Edits autosave

Autosave as you type, and a saved plan is never re-seeded.

A signature you can hand an auditor

Role-bound link, content hash, expiry and a certificate page appended to the PDF.

Approvals carry a reason

Approver seats sign with a written justification; attachments ride along in the PDF.

Guardrails

What it will never do.

E-signature integrity (hash, certificate, expiry); approver justification; signed weekly sheets for audits. How we handle data

Built into the engine, the same for people and agents

  • Never re-seeds a saved plan · Your edits survive every regeneration.

  • One link, one signer · Role-bound, and dead after 30 days.

  • Hashes what was signed · SHA-256 over exactly what was shown.

  • Never approves silently · Every approver sign-off carries a written reason.

Who writes this plan

One of the staff drafts it for you.

The Care Plan Generator seeds the plan from the authorization's service code and pushes tasks into shifts.

It drafts. A human reviews and signs every plan, and it never gives clinical advice.

Care Plan Generator · From authorization to a signed plan.

  1. Tue

A T1019 authorization lands. The plan seeds from your task library.

  2. Wed

The case manager edits two tasks and sends both signature links.

  3. Thu

Both signatures in. Status flips, certificate attaches, Friday's shift is loaded.

What is home care care plan software?

Care plan software turns an assessment and authorization into a plan the caregiver actually follows at the visit. In Nestaid the plan is keyed to the billing service code, its tasks appear in each shift as day-exact suggestions, and client and case-manager e-signatures are captured with a content hash, expiry and certificate to support audit review.

Frequently asked questions

Do tasks really show up in the shift?

Yes. Plan tasks are suggested into each shift for that client on that day, and the caregiver checks them off in the app.

Who can sign?

The client (or representative) and the case manager, through role-bound links; supervisor approver seats sign with a written justification.

What if the plan is edited after signing?

Re-sending supersedes the earlier request; status is derived from the current signatures.
